I confirm I understand and agree to this declaration.

By submitting this Waste Declaration, I (the undersigned) declare and confirm that:

(1) The information provided in this Waste Declaration (including all attachments) is true, complete, accurate and not misleading, and I acknowledge Repurpose It Pty Ltd (Repurpose It) will rely on this declaration when deciding whether to approve acceptance of the waste. I acknowledge that this declaration forms part of the contractual and regulatory basis upon which Repurpose It accepts waste, and that providing false, misleading or incomplete information may result in refusal of waste, cancellation of approval, regulatory reporting and recovery of costs.

(2) I am the waste producer, or I am authorised by the waste producer to manage the waste described;

(3) all waste delivered to Repurpose It Pty Ltd (Repurpose It) will originate only from the nominated source address and the specific stockpiles/domains identified in this
declaration;

(4) the soil/waste has been classified in accordance with Regulation 66 of the Environment Protection Regulations 2021 and supporting evidence is attached;

(5) the waste does not contain asbestos or asbestos-containing material;

(6) Category A Reportable Priority Waste (RPW), waste containing asbestos, and any other non-permitted waste will not be delivered to Repurpose It;

(7) every load of RPW will be accompanied by a valid EPA Waste Record and all applicable EPA tracking and transport requirements will be complied with;

(8) any transporter engaged or nominated by the waste producer to deliver the waste is lawfully authorised to transport the relevant waste type and complies with all requirements of the Environment Protection Act 2017, the Environment Protection Regulations 2021 and applicable waste transport and tracking obligations;

(9) Repurpose It may refuse entry to, reject (including at the weighbridge), quarantine
and/or require removal of any load it reasonably considers unsuitable or non-compliant;
and

(10) the waste producer indemnifies Repurpose It against all losses, costs, expenses,
liabilities, penalties, clean-up, removal, transport, disposal, testing and associated
costs to the extent arising from or in connection with any waste delivered that is
inconsistent with this declaration or any approval given by Repurpose It, contains
asbestos or asbestos-containing material not expressly disclosed and approved in
writing by Repurpose It, or any act or omission of any transporter engaged or nominated
by the waste producer (including where the transporter is not lawfully authorised or fails
to comply with applicable transport or EPA tracking requirements), including, without limitations, costs of plant and machinery, labour, disposal fees, and loss of income resulting from site restrictions or closures.
